CONSENT TO USE PERSONAL INFORMATION AND LIKENESS

Pursuant to Section 32(b) of the Freedom of Information and Protection of Privacy Act

(RSBC 1996, c. 165) I give my consent to Simon Fraser University (“SFU”) to use my personal information, including photographs, video recordings and sound recordings of me under the terms and conditions noted below.

1. The personal information to be used is as follows:
My name and likeness in the form of photographs, videos and sound recordings.

2. The personal information may be used by the following SFU employees:
SFU employees tasked with creating illustration, promotion, art, exhibition, editorial, advertising, broadcast, trade, educational or any other work related to SFU’s activities.

3. The personal information and likeness is used only for the following purpose(s):
Illustration, promotion, art, exhibition, editorial, advertising, broadcast, trade, educational and other purposes related to publicizing SFU’s activities, programs, presentations, ceremonies, performances, sports meets and similar events.

I further grant to SFU the absolute and irrevocable right and unrestricted permission to use, reuse, publish, and republish in any format, location or medium at any time such photographs, video recordings and sound recordings (“the Material”). I further grant to SFU the right to edit the Material before or after use, without restriction. I understand and agree that the Material and all worldwide copyrights therein shall be the sole and exclusive property of SFU, free of all claims by the undersigned or any other party. I understand and agree that this permission applies to the Material in whole or in part, in any and all media now or hereafter known, for purposes mentioned above. I understand and agree that SFU may identify me by name in any use or reuse of the Material in which I appear or am recorded. I hereby release and discharge SFU and its employees and agents from any and all claims and demands arising out of or in connection with the use or reuse of the Material, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

4. This consent to use my personal information is permanent. SFU will use the photographs,
videos and sound recordings for the purposes noted at point 3) above for a period of 5 years, during which, the visual records of me could be available on the SFU Image Library (www.sfu.ca/image- library). After visual and sound recordings are no longer needed for the purposes noted above, they may be transferred to the SFU Archives for permanent retention.

SFU reserves the right in its sole discretion to use or not use the personal information, including photographs, at any time without any notice to me. I acknowledge that I am required to inform my SFU contact (the person who asked me to provide this Release) of any circumstances, when I become aware of them, that may adversely affect my reputation and standing in the community.

No use of personal information about me beyond the terms described above is permitted without my express written authorization or unless permitted by law.

I am of full legal age and have the right to contract in my own name. I have read the foregoing and fully understand the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.
