Accessible Booking Request

Accessible Appointment Scheduling

Are you deaf or hard of hearing? Do you have accessibility needs or challenges using the phone? 

  • Book an appointment with a Counsellor or Psychologist by emailing us at moa@hcs1.sfu.ca
  • Submit a medical appointment request with a Doctor, Registered Nurse, Mental Health Nurse or the Transitional Case Manager by completing the form below and emailing it to moa@hcs1.sfu.ca

Appointment requests are NOT confirmed until the medical office assistant contacts you via email within 2 to 3 business days.
