From Wade Parkhouse | Upload proof of vaccination by November 5th

A reminder that, even if you have already declared your vaccination status, you must also now upload proof. The deadline for proof of vaccination is November 5.
The easiest way is to upload a photo of your BC Vaccine Card to our dedicated Thrive website. There are step by step instructions on our dedicated vaccine declaration page. There is also a fillable form on the page if you’d prefer that method of providing proof. The form can be returned to covid19@sfu.ca, or if you prefer it can be placed in an envelope and dropped off at Human Resources, Faculty Relations, or Student Central on the Burnaby Campus, or at Registrar and Information Services on the Surrey or Vancouver campuses. We will begin auditing proof of vaccination after the November 5 deadline.
Declaration of vaccine status is mandatory for all students, faculty and staff. We are now following up directly with supervisors/leadership across the institution to ensure all faculty and staff have declared, and Student Services is undergoing a parallel process with our students. This one-on-one approach has been proven effective in achieving the highest possible rate of declaration. If you still have not done so, please take a moment to declare your vaccination status now.
