Forms

These are for documenting student records or to simplify processes by ensuring all required information is provided with a request. Records and processes needed for all graduate students across the University are linked from the Graduate Studies website. All completed forms should be submitted to the Graduate Program Assistant prior to getting REM Graduate Program Chair approval unless clearly indicated on the form.

REM Directed Studies Form
Complete this form and attach a course outline from the course instructor if you wish to complete a Directed Studies.

Course Waiver Form
If a course waiver is approved by the course instructor, this form needs to be provided to ensure it is recorded on your Academic Progress Report (APR).

PhD Course Planning form
For PhD students only, to be completed in conjunction with Sr. Supervisor. in the first year of the program. Records any course substitutions or academic plans.

REM 898 PhD Thesis Proposal Examining Committee form
Submit prior to defending REM 898 Thesis proposal by the end of the fifth term of the PhD program.

REM TARA Supplimentary Application form
Refer to the Terms of Reference for this award, then complete the form and your supervisor must review and approve the expenses. Include in the supplimentary documents uploaded into GA3 with your TARA application.
