PARTICIPANT INCLUSION INFORMATION FORM
This form is intended for parents/guardians/caregivers to provide information to the Camp Services team to help us best support participants with exceptionalities, additional needs, or disabilities.
SFU Camps Inclusion Policy
SFU Camps is committed to creating a safe, welcoming, and inclusive environment to all camp participants regardless of exceptionality or behaviour. By providing us with additional information prior to the start of camp, we can create a collaborative environment and work together to provide the resources and support to set all campers up for success. We invite you to share as much information and details as possible; this will provide staff more opportunities to adapt their programming and be confident in supporting your participant.
All the information you share through the Participant Inclusion Form will remain confidential and only be shared with the Camp Administrative team and the staff working directly with your child.
**Please submit form a minimum of 2 business days before attending camps to ensure information is received and communicated to relevant staff**
Please Note: While our staff works hard to adapt camp programming for participants with exceptionalities, SFU Camps is unable to provide assistance with toileting, changing, lifting or carrying, or administering medication. Should your child require this type or similar assistance, we kindly ask that a parent, caregiver, or family member be available to provide care and support when needed.
Please Note: At this time, SFU Camps is unable to provide dedicated 1-1 support to campers. When resources allow, we are happy to place an additional volunteer with the camp. All of our volunteers have undergone a Criminal Record Check and completed our Camps training program.
