Applying for Accommodation on Medical Grounds

Students who are unable to complete course work on time or who cannot write an examination at the regularly scheduled time for medical reasons need to provide the instructor with official medical documentation showing why they were unable to complete the required work at the scheduled time. Simon Fraser University has developed a form specifically for this purpose. A link to the university's form that students should use to apply for accommodation in their classes for medical reasons is provided here.

Note that this form must be printed by the student, completed by the health care provider, and submitted to the instructor. Use of this form does not guarantee that the student will be accommodated; however, it is in the student's interest to have the form completed by the health care provider as soon as the medical condition manifests itself, and submitted (in person) to the instructor as soon as the student is well enough to do so.

In addition, the student is advised to contact the instructor as early as possible (by email) indicating that they will be unable to complete the task at hand on time and that they are obtaining the appropriate documentation. (Be specific in your email. Be sure to include your full name and student number, the name of the course, the date(s) at issue, and so on.)
