Self-Management of Mood Problems for People with Chronic Diseases Workbook
The purpose of this project was to develop a workbook for self-management of mood problems in individuals suffering from chronic diseases (including chronic obstructive pulmonary disease and chronic heart failure, diabetes, and renal dysfunction). This workbook drew upon the knowledge and expertise of rehabilitation staff in Vancouver Coastal Health Authority, as well as expert researchers and clinicians regarding crucial issues to address and optimal approaches to self-care among chronically ill patients. It represents a tool that would be complementary to the training delivered by the British Columbia College of Family Physicians Office-Based Self-Management Program. The workbook is based on well-established cognitive-behavioural change methods, integrating behavioural skills that help reduce mood problems in combination with those that foster improved self-care and illness management.
