Name, address, email address, telephone number, date of birth, sex, pronouns, gender identity, ethnicity, language spoken, disability, marital status, family status, student number, client status, personal health number, third-party insurance number, emergency contacts, employer contacts, medical history.
The University collects and uses the personal information contained in these records to administer and manage employees’ sick leave benefits and required medical accommodations arising from occupational and/or non-occupational illness/injury. Activities documented include (but are not limited to) reaching out to emergency contacts in cases of need, using correct gender nomenclature in paperwork and interactions, scheduling leave (and the associated coverage), and distributing benefits.
Staff members of Human Resources, Faculty Relations, and eligible employees' home departments may use the information.
