Key Dates
- Course Registration Deadline: Monday, January 31, 2005.
- Registrations
for students who have previously attended the course will be processed
after Monday, January 31, 2004 provided that space is available.
Course
Fee
The course fee is $255 CDN/ $220 US and includes all sessions, course
materials, refreshment breaks, and a light buffet reception and social
hour (cash bar)
on Monday, February 28th. The US dollar fees may be subject to change
depending on currency fluctuations.
Parking
Parking is available on campus for $15 CDN / $13 US (Monday–Friday)
payable in advance with course registration. Passes will be mailed out
to registrants.
Banquet
The banquet will be held on Thursday evening, March 3. Venue to be
advised. Tickets are sold at cost: $30 CDN / $26 US per person.
Cancellations
and Refunds
Written requests for refunds should be received by Conference Services
by Monday, February 7. Refunds are subject to a $30 CDN / $26 US administration
charge.
Registration may be transferred to another individual; please advise
Conference Services in advance of such a change. No refunds will
be issued to participants
who fail to attend the course without prior notice.
How to Register
On-line
Complete the ON-LINE
Form. You will be given the option of paying
for your registration fees on-line via a secure server (Visa or
MasterCard). Sorry,
Simon Fraser University
does not accept American Express or Diners Club.
All credit card transactions will be processed in Canadian dollars
and are subject to current exchange rates.
By Mail or Fax
Complete the ON-LINE Form and mail with your cheque or bank draft (in
US or Canadian dollars) made payable to Simon Fraser University to
the address provided below. Forms can also be faxed and cheques mailed
separately. Note: Faxed or mailed registrations will not be processed
until payment is received.
Confirmation of your registration
If you register on-line and pay by credit card, you will receive
immediate confirmation via our server.
Receipts
If you pay on-line by email transaction (MasterCard of Visa), you will
be shown your
confirmation receipt on-line (while you are still logged in), please
print out this receipt as this will be the original copy. If you
pay by
cheque, you will
be given your
receipt
at the conference registration desk.
